2018 Flu Shots and Health Fairs
Even healthy people can get the flu!
You MUST complete an insurance form and provide the information on your medical insurance card to get a flu shot. If you are not enrolled in a City Medical Plan but you ARE a City employee, you are still welcome to get a Flu Shot (at no cost).
Save Time: Complete the FLU SHOT FORM in advance to ensure a quicker line and process.
Who can’t get a flu shot: These locations are servicing City employees only and cannot accommodate family members. Family members should visit their doctor or insurance participating pharmacy for a free flu shot.
