2017 Flu Shots and Health Fairs
Even healthy people can get the flu!
You MUST complete the FLU SHOT FORM and provide the information on your medical insurance card to get a flu shot: This enables the vendor to bill either Kaiser Permanente or Regence.
Important Note: If you are not enrolled in a City Medical Plan, because you are a City employee, you are still welcome to get a Flu Shot (at no cost). You do still need to complete the insurance form (but you can leave the medical section blank).
Save Time: Complete the FLU SHOT FORM in advance to ensure a quicker line and process.
Who can’t get a flu shot: These locations are servicing City employees only and cannot accommodate family members. Family members should visit their doctor or insurance participating pharmacy for a free flu shot.
